Aloha and welcome to the 2021-2024 Veggie Rx Program.
Enrollments in the program are conducted through participating clinics.
Limited spaces remain for 2023-2024 through participating clinics.

The form includes core enrollment information, and then links to an intake survey.
Once completed, submitted, and approved as eligible, the participant will be sent information on how to begin ordering their Veggie Rx produce.
Note: Please be sure to save the forms if you or the patient need to return and complete it later. Saving a form allows you to email a link to it to your staff email and to then return and complete later.
Forms are stored on a HIPAA compliant server to protect patient privacy.
